To play Cthulhu Realms, each player starts with a small deck of initiates, goons, and followers. The table is set with five available cards, or three when playing with more than two players. On each turn, players strategically use their cards to place locations into play, boost their sanity, reduce their opponent's sanity, collect conjuring power for new cards, and remove cards from the game. The power of locations can be utilized once per turn. At the end of each turn, players discard any remaining cards and remove entities and artifacts played.

The objective of Cthulhu Realms is to either be the last player with sanity or have the most sanity when the deck runs out. Losing all sanity eliminates a player from the game, while being the sole sane player declares victory.
